Reduce Medicaid Costs - Designated Provider Zones are dense clusters of >65 MassHealth members (300 or more) centered around elder housing/apartment buildings who are assigned designated providers. All members are less than a 10 minute walk. DPZ’s reduce duplicated services and substantially reduce provider travel time: Travel time = provider $/hour = wasted $. DPZ’s require minimal staffing:  ½ day/four days a week.   They provide enhanced management capabilities for improved oversight of transition of care & continuous complex patients, and eliminate often difficult and taxing efforts by members to see their PCP -  resulting in less untreated exacerbated conditions and less ED visits.
